Greg Kuzma, MPT

Greg Kuzma is the owner of Ashley and Kuzma Therapeutics. Greg is from Erie, PA. He graduated from Duke University in 1990 with a bachelors degree and then from Slippery Rock University in 1993 with a masters degree in Physical Therapy.

Greg has worked in many different settings, which include 9 years working for Hamot Medical Center, 7 years as a Rehab Advisor for the Total Joint Replacement Committee, and 15 years in the private practice setting. In 2002, Greg opened the clinic with his former partner Mike Ashley, PT who has now retired. Over the years, Greg has been active outside the clinic coaching his sons wrestling, golf and baseball. He feels that his coaching experiences and personal rehabitation experiences from Rotator Cuff and Knee injuries helps him to better guide his patients towards recovery. Greg has also been McKenzie Trained to treat lower back pain and is a member of the American Physical Therapy Association. 

Bethany Sivak, PT, DPT

Dr. Bethany Sivak grew up in Millcreek and received a Bachelor’s of Natural Science in 2018 and Doctorate of Physical Therapy in 2020 from Daemen College in Buffalo, NY. She began working for Ashley & Kuzma Physical Therapy after graduating in August 2020. Bethany has experience in pediatric and outpatient orthopedic and neurologic rehabilitation. When she’s not working, she enjoys spending time with family and friends, traveling, and spending time at Presque Isle when the weather is nice.

Sierra Iavarone, PT, DPT. AIB-VRC

Dr. Sierra Iavarone grew up in Erie and graduated from Daemen College with her Bachelor’s in Natural Science in 2018 and Doctorate of Physical Therapy in 2020. She played sports growing up and was introduced to the field of physical therapy through sports injuries. Sierra has an interest in outpatient physical therapy and neurologic rehabilitation. She has experience in pediatric and acute/sub-acute rehabilitation. Sierra also enjoys photography in her down time. Sierra has completed training in vestibular rehabilitation through the American Insititue of Balance and can help treat patients with vertigo (BPPV), concussion, and conditions affecting the inner ear.

Tyler Kuzma, PT, DPT, AIB-VRC

Dr. Tyler Kuzma went to Fort LeBoeuf schools and later earned his Bachelor’s in Exercise Physiology from West Virginia University in 2018 and his Doctorate of Physical Therapy from Gannon University-Ruskin Campus in 2021. Tyler started wrestling when he was young and went on to wrestle for Fort LeBoeuf High School. He was part of the wrestling team while they won 3 State medals. He has experience in outpatient and inpatient rehabilitation settings. Tyler continues to stay active in the gym and enjoys traveling. Dr. Tyler has completed vestibular rehabilitation certification through the American Institute of Balance and can help treat conditions like vertigo and concussion.

Andrea Costello, PT, DPT

Dr. Andrea Costello graduated from Gannon with a Bachelor’s in Sport & Exercise science in 2007 and her Doctorate of Physical Therapy in 2010. She worked in hospital based outpatient orthopedics in Coudersport, PA for 8.5 years and outpatient orthopedics in Rocherster, NY for 4.5 years before joining our team. She is originally from Corry and moved to Erie to be closer to family. Andrea enjoys spending time with family and friends, traveling to new places and hiking in national parks, and doing photography and scrapbooking.

Lindsey Reid, PTA

Lindsey was raised in Girard and went on to graduate from Mercyhurst Northeast with her Associate of Science as a Physical Therapist Assistant in 2006. She has experience in acute care, geriatric care, and aquatic therapy settings. Lindsey stays busy at home with her son and a set of twins. She enjoys spending time with her family and getting outdoors to hike or kayak when the weather is nice.

Amanda Durst, PTA

Amanda graduated from Mercyhurst Northeast with an Associate of Science as a Physical Therapist Assistant in 2012. She was raised in Harborcreek and spent time living in Texas before moving home to Erie and starting her family. Amanda has experience in home health, inpatient, and outpatient settings. She loves spending time with her family and taking her two daughters on adventures!

Amanda Fadely, PTA

Amanda graduated from University of Pittsburgh with a Bachelor’s Degree in Sports Medicine in 2006 and Mercyhurst Northeast for an Associate of Science as a Physical Therapist Assistant in 2009. Amanda has worked in outpatient orthopedic settings as a PTA and was at AKPT previously before recently finding her way back! She enjoys spending time with her family, camping, and crafting in her free time.

Frank DiSalvo, Physical Therapy & Phototherapy Technician

Frank graduated with a Associates degree in graphic design from the Art Institute of Pittsburgh. He has past experience in many different health care settings including 10 years at Hamot Medical Center in their Behavioral Health Department, 3 years in the Cast/Wound Clinic at Hamot Medical Center, and 7 years working in the private practice outpatient setting. Frank has 35 years experience in martial arts and also works with several of the local schools in support of anti-bullying programs. He also enjoys cycling and being in the outdoors.   

Sarah Young, Physical Therapy & Phototherapy Technician

Sarah Young grew up in Waterford, PA. She has worked at Fox Run Golf Course and has been a technician at Ashley & Kuzma since 2020. Sarah quickly picked up on helping with physical therapy and phototherapy and is a wonderful asset to our clinic! She enjoys reading and spending time with friends and family. Sarah loves to see the good in others and meet new people!
